Resize Text	Care Coordination Program The AAAHOT Care Coordinator assesses needs of older persons and effectively plans, arranges, coordinates and follows-up on services which most appropriately meet the needs identified by client, family members, caregivers and Care Coordination staff. Services include: Home repair/modification services for low-income homeowners over age 60 that are living in unsafe/unhealthy environments. Primary focus is on repairs/ modifications that improve the accessibility, structure, safety and weatherization of the home. Health maintenance services for persons over age 60 that includes the provision of durable medical equipment that will prevent, alleviate and/or cure the onset of acute and/or chronic illness.
